Medicare Weight Loss Medication Update

Beginning July 1, 2026, eligible Medicare Part D beneficiaries may be able to access certain GLP-1 weight loss medications through the new Medicare GLP-1 Bridge Program. This short-term CMS demonstration program is designed to help qualifying patients receive covered obesity treatment medications with a $50 copay.

For many Medicare patients, access to prescription weight loss medications has been limited, confusing, or difficult to navigate. The Medicare GLP-1 Bridge Program creates a temporary pathway for eligible beneficiaries who are prescribed GLP-1 medications specifically to reduce excess body weight and maintain weight reduction.

Who Qualifies for the Medicare GLP-1 Bridge Program?

To qualify for the Medicare GLP-1 Bridge Program, a provider must submit a prior authorization request confirming that the beneficiary meets CMS eligibility requirements. Patients must be prescribed the medication specifically for weight reduction and weight maintenance while participating in ongoing lifestyle modification, including nutrition and physical activity programs.

CMS established three primary eligibility pathways based on body mass index (BMI) and certain obesity-related health conditions.

Important: Beneficiaries must be at least 18 years old and use the medication for weight management purposes. Prior authorization approval is required before medications can be dispensed through the program.

Medicare GLP-1 Bridge Eligibility Requirements

Option 1

BMI 35 or Higher

Beneficiaries may qualify if they have a body mass index (BMI) of 35 or greater at the time they begin GLP-1 therapy.

  • Age 18 or older
  • BMI ≥ 35
  • Weight management indication
  • Ongoing nutrition and physical activity program
Option 2

BMI 30 or Higher With Certain Medical Conditions

Beneficiaries with a BMI of 30 or greater may qualify if they have at least one qualifying obesity-related condition.

  • Heart Failure With Preserved Ejection Fraction (HFpEF)
  • Uncontrolled Hypertension
  • Chronic Kidney Disease (Stage 3a or higher)
Option 3

BMI 27 or Higher With Cardiometabolic Risk Factors

Beneficiaries with a BMI of 27 or greater may qualify if they have one of the following conditions.

  • Prediabetes
  • Previous Heart Attack (Myocardial Infarction)
  • Previous Stroke
  • Symptomatic Peripheral Artery Disease (PAD)

Who May Not Qualify for the Medicare GLP-1 Bridge Program?

One of the most misunderstood aspects of the Medicare GLP-1 Bridge Program is that it was specifically created for Medicare beneficiaries seeking treatment for obesity and weight management.

CMS has stated that beneficiaries who already have access to GLP-1 medications through Medicare Part D for certain FDA-approved indications generally are not eligible for the Medicare GLP-1 Bridge Program.

Patients Typically Directed to Medicare Part D Coverage

  • Type 2 Diabetes
  • Obstructive Sleep Apnea (OSA)
  • Noncirrhotic Metabolic Dysfunction-Associated Steatohepatitis (MASH)

If a Medicare beneficiary already qualifies for GLP-1 coverage through Medicare Part D for one of these conditions, the medication should generally continue to be obtained through their Part D prescription plan rather than the Medicare GLP-1 Bridge Program.

Which Medications Are Covered Under the Medicare GLP-1 Bridge Program?

Beginning July 1, 2026, eligible Medicare beneficiaries may obtain certain FDA-approved GLP-1 medications through the Medicare GLP-1 Bridge Program when prescribed specifically for weight reduction and weight maintenance.

CMS updated the program on April 6, 2026, to include Foundayo® following FDA approval and to clarify which Zepbound® formulations are eligible.

How to Apply for the Medicare GLP-1 Bridge Program

Medicare beneficiaries cannot simply request a GLP-1 medication through the Bridge Program without clinical review. A prescribing provider must evaluate the patient, confirm that CMS eligibility criteria are met, and submit a prior authorization request.

The process usually begins with a medical weight loss consultation, BMI review, medication history, health condition review, and insurance verification.

Prior Authorization Requirements

Prior authorization is required for the Medicare GLP-1 Bridge Program. This means the prescribing provider must submit information showing that the medication is medically appropriate and that the patient meets CMS eligibility rules.

Why Medicare GLP-1 Bridge Requests May Be Denied

Not every patient who wants Wegovy® or Zepbound® will qualify. CMS eligibility rules are specific, and prior authorization requests may be denied if the required criteria are not met.

Patients below the required BMI threshold may not qualify for the Bridge Program.

No Qualifying Condition

Patients with BMI 27–34.9 may need a qualifying condition depending on their BMI level.

Medication Not Included

Ozempic®, Mounjaro®, and some Zepbound® formulations are not included in the program.

Part D Indication Applies

Patients with Type 2 diabetes, obstructive sleep apnea, or MASH may be directed to Medicare Part D coverage instead.
