How the Tirzepatide and Zepbound® Calculator Works

This tool converts a prescribed tirzepatide dose in mg into liquid volume in mL. When a clinician or pharmacy has specifically instructed you to use a U-100 syringe, it also shows the corresponding syringe units.

The calculation uses the concentration printed on the vial: dose ÷ concentration = mL. For a U-100 syringe, mL × 100 = units. A 5 mg dose at 10 mg/mL, for example, equals 0.5 mL or 50 U-100 units.

Zepbound® prefilled pens are single-dose devices and should be used exactly as directed. This vial calculator is not intended to measure, divide, or transfer medication from a prefilled pen.

Why Units Can Be Confusing

Many patients assume that a certain dose always equals the same number of units. That is not always true. A 2.5 mg dose of tirzepatide may equal one number of units at 5 mg/mL and a different number of units at 10 mg/mL or 20 mg/mL.

This is especially important for pharmacy-dispensed vials that may be prepared at different concentrations. Do not use another patient’s instructions, an old label, or a general chart in place of the directions provided with your medication.

How do I convert tirzepatide from mg to units?

First divide the prescribed dose in mg by the medication concentration in mg/mL. This gives the volume in mL. Then multiply the volume by 100 to estimate units on a U-100 insulin syringe.

Why can the same dose equal different numbers of units?

The number of units depends on the medication concentration. A 2.5 mg dose may equal a different number of units if one vial is 5 mg/mL and another is 10 mg/mL or 20 mg/mL.

Can this calculator be used for a tirzepatide vial?

It can provide a mathematical estimate when the vial label clearly states the tirzepatide concentration in mg/mL and your clinician or pharmacy has instructed you to measure the dose with a U-100 syringe. It is not for Zepbound® prefilled pens.

How many weeks will one vial last?
